ಚಿಕಿತ್ಸಾತ್ಮಕ ಮನೋಶಾಸ್ತ್ರ
Psycho oncology is an interdisciplinary area bringing together mental health and oncological fields to analyse the psychological, behavioural and social aspects of cancer. Cancer is an illness that comes with an unavoidable amount of distress from the beginning of its diagnosis further continuing into the treatment stages, and at the palliative care stage. It is important to look into the emotional, social and behavioral changes that occur in a patient and their caregiver’s lives which will need psychological support from professionals such as psychiatrists, clinical psychologists, psychological counselors, and psychiatric social workers. Providing cancer patients and caregivers with coping strategies, adjustment techniques, stress management, pain management, relaxation techniques, and other emotional support. Supporting psychological wellbeing will not only improve the quality of life but also help the patient in having a more positive outlook and optimistic attitude towards the process and treatment.
ಚಿಕಿತ್ಸಾತ್ಮಕ ಮನೋಶಾಸ್ತ್ರ ಘಟಕ ಒಂದು ರೆಫರಲ್ ಘಟಕ (ಯೂನಿಟ್) ಆಗಿದೆ. ಈ ಘಟಕವು ಒಬ್ಬ ಚಿಕಿತ್ಸಾತ್ಮಕ ಮನಶ್ಶಾಸ್ತ್ರಜ್ಞರನ್ನು ಮಾತ್ರ ಒಳಗೊಂಡಿದೆ. ಚಿಕಿತ್ಸಾತ್ಮಕ ಮನೋಶಾಸ್ತ್ರ ಘಟಕವು ಪ್ರಾಥಮಿಕವಾಗಿ ಕ್ಲಿನಿಕಲ್ ಆಗಿದ್ದು, ಇದು ಕ್ಲಿನಿಕಲ್ ಸೇವೆಯಾಗಿದೆ. ಆದಾಗ್ಯೂ, ಹೆಚ್ಚಿನ ಶೈಕ್ಷಣಿಕ ಮತ್ತು ಸಂಶೋಧನಾ ಚಟುವಟಿಕೆಗಳಿಗೆ ಅವಕಾಶಗಳು ಇವೆ. ಮಾನಸಿಕ ಮೌಲ್ಯಮಾಪನ ಮತ್ತು ನಿರ್ವಹಣೆಯ ಅಗತ್ಯವಿರುವ ರೋಗಿಗಳನ್ನು ಲಿಖಿತ ಶಿಫಾರಸ್ಸಿನ ಮೂಲಕ ಕ್ಲಿನಿಕಲ್ ಸೈಕಾಲಜಿಸ್ಟ್ ರನ್ನು ಭೇಟಿಯಾಗಲು ಹೇಳಲಾಗುತ್ತದೆ. ರೋಗಿಗಳು (ಮಕ್ಕಳು/ ವಯಸ್ಕರು, ಹೊರರೋಗಿಗಳು, ಒಳರೋಗಿಗಳು, ಪುರುಷರು/ ಮಹಿಳೆಯರು ಹೀಗೆ ಕ್ಯಾನ್ಸರ್ ಅಂದರೆ ನಿರಂತರವಾಗಿ ಎಲ್ಲಿಂದಲಾದರೂ ಶಿಫಾರಸ್ಸುಗೊಳ್ಳಬಹುದು) ತಪಾಸಣೆಗೂ ಮುಂಚಿನಿಂದ ಆರಂಭಿಸಿ ಮರಣದ ಹಂತದವರೆಗೆ ಮಾನಸಿಕ ಬೆಂಬಲ ಅಥವಾ ಸಹಾಯದ ಅಗತ್ಯವಿರುವ ರೋಗಿಯನ್ನು ಗುರುತಿಸುವ ವಿಧಾನಗಳು ಹಲವು ಇವೆ. ಸ್ವತಃ ರೋಗಿಯ ಮೂಲಕ, ಕುಟುಂಬದ ಸದಸ್ಯರ ಮೂಲಕ, ಶುಶ್ರೂಷಾ ಸಿಬ್ಬಂದಿ ಅಥವಾ ಚಿಕಿತ್ಸೆ ತಂಡದ ಯಾವುದೇ ಸದಸ್ಯರ ಮೂಲಕ ಸಹಾಯದ ಅಗತ್ಯವಿರುವ ರೋಗಿಯನ್ನು ಗುರುತಿಸಲಾಗುತ್ತದೆ. ಆದಾಗ್ಯೂ, ಲಿಖಿತ ಶಿಫಾರಸ್ಸನ್ನು ಚಿಕಿತ್ಸೆ ನೀಡುತ್ತಿರುವ ವೈದ್ಯರು ತಯಾರಿಸುತ್ತಾರೆ.
SCOPE OF SERVICES
To provide psychological screening to the patients and family members/caregivers for psychological concerns and distress.
To deliver psychological intervention and counselling services to all the patients and care-givers from various departments at KMIO
To conduct in depth psychological assessments for various psychological comorbidities.
To provide pharmacotherapy for patients and caregivers with psychiatric comorbidities.
To provide counselling services and psychological support for staff burnout.
Management of pain, fatigue, anticipatory nausea and vomiting etc is also provided.
To conduct counselling services for patients and caregivers who are part of the after treatment completion clinic in the pediatric oncology department.
Bereavement and grief counselling for those in palliative or end of life care.

CLINICAL SERVICE : It is the most important service of the department. The Psycho-oncology services cater to the patients of various age groups ranging from children to adults, caregivers, staff and other stakeholders. The department follows a care model which is explained below.
PSYCHO ONCOLOGY MODEL OF CARE

SCREENING
To find out psychiatric comorbidities and improving the outcomes of cancer.
It can be administered by resident doctors, nurses, social workers, counselors, clinical psychologist, and psychiatrist.
Distress Thermometer (DT) and Patient Health Questionnaire (PHQ) are used as two major screening tools.
UNIVERSAL CARE
The Universal care step is provided for patients with Minimal level of distress.
The professionals involved in this stage of care are Treating clinicians, Resident Doctors and Nurses.
They can provide support to patients in the form of Patient education and Brief emotional support.
SUPPORTIVE CARE
The supportive care step is provided for patients with Mild levels of distress.
The professionals involved in this stage of care are Resident Doctors, Nurses and Social Workers.
Patients can be supported by them in the form of Psychoeducation, Social support, and Supportive counselling for physical concerns.
EXTENDED CARE
The extended care step is provided for the patients with moderate levels of distress.
The professionals involved in this are counsellors and clinical psychologists.
Support is provided to patients in the form Psychoeducation, Counselling sessions, Mindfulness, Coping skills training and Group therapy.
SPECIALIST CARE
Specialist care is provided to patients with severe level of distress.
Clinical Psychologist and Psychiatrist are involved in this stage of care, providing interventions such as Psychotherapy and Pharmacotherapy.
ACUTE CARE
Acute care is provided for patients with extreme severe levels of distress where if required admission as an in-patient, treatment is provided.
As it is a long term plan Psychiatrist and clinical psychologist are involved in this stage of care.
Patients may show adjustment disorder, anxiety, depression, delirium and personality disorders with high level of intensity which is comorbid with cancer.
The interventions provided as in specialist care such as psychotherapy and pharmacotherapy with the need of more care.
ACADEMIC PROGRAMS : The department conducts various academic sessions, journal club and case presentations within the department on topics like :

Psychosocial assessment in the pediatric cancer care
Impact of depression on cancer patients
Psychoeducation and anticipatory guidance in pediatric cancer care
Interdisciplinary collaboration of psychosocial care for pediatric cancer
Monitoring & Assessment of Neuropsychological outcomes as a standard of care in pediatric oncology
Psychosocial Follow up in Survivorship as a standard of Care in Pediatric Oncology
Psychosocial Interventions and Therapeutic Support as a Standard of Care in Pediatric Oncology
Assessment of Financial Burden as a Standard of Care in Pediatric Oncology
Standards of Psychosocial Care for Parents of Children With Cancer
The team also conducts training programs for other departments on different topics associated with psycho oncology.

LIST OF FOCUSSED AREAS
Integrated Pediatric Psycho Oncology Services : This includes services for the pediatric patients and their primary caregivers. All patients in the IP and their families are given bed-side psychological support, OPD patients are met on referral basis. Psychological services are also provided in the After Treatment Completion clinic to patients and caregivers. Services provided are :

Integrated Palliative Psycho Oncology Services : The services provided are for patients and their family members. All IP and OP patients are met in the palliative department.
